Overview

Tight junctions establish the paracellular barrier that controls the flow of molecules in the intercellular space between the cells of an epithelium [1][2]. Claudins are crucial components of tight junctions. They are small (20-27 (kDa)) [3] transmembrane proteins containing typically four transmembrane domains and two extracellular loops (figure 1). The claudin family consists of approximately 27 proteins that locate differentially in different tissues. Their altered function has been linked to various cancers [4], which makes them potential targets for diagnostics and therapeutic modalities.

Claudin 18.2 is the isoform 2 of Claudin-18 protein. Its expression in normal tissues is strictly confined to differentiated epithelial cells of the gastric mucosa [5]. However, CLDN18.2 is highly expressed in certain primary malignant tumors, such as gastric cancer (GC) [6], breast cancer, colon cancer and liver cancer. Its expression is also present in pancreatic, esophageal, ovarian, and pulmonary tumors. Its abnormal expression in various epithelial tumors indicates that CLDN18.2 may play an important role in tumor invasion and metastasis.

CLDN18.2 is often linked with proliferation and chemotaxis of cells in gastric cancer. According to Globocan 2020 data, gastric cancer is the fifth most frequently diagnosed cancer, over one million new cases of gastric cancer are diagnosed worldwide. Gastric cancer is a heterogenous disease characterized by expression of specific proteins, including MUC17, CLDN18.2, receptor tyrosine kinases (e.g., HER2), and growth factors (e.g., VEGF) [7, 8, 9]. CLDN18.2 is expressed in 50 - 70% of primary gastric adenocarcinomas and their metastases, thus it is considered a promising candidate for targeted GC therapy [10].
